Forms
- Appeal Form for Readmission (After Dismissal) -- this form is completed and signed by the student then submitted to the Scholastic Standing Committee
- Application for Certification of a Minor -- this form is approved and signed by the department chair and returned to the academic advising office, which then conveys the information to the Registrar’s Office.
- Application for Inter-College Registration -- this form is approved and signed by the dean
- Application for Permission for Transient Work -- this form requires three signatures: (1) the dept. chair of the student's major department (2) the chair of the department teaching the course and (3) the associate dean of CCAS.
- Diploma Application -- this form is completed by student then submitted to the advising office on the second floor of Armitage Hall
- Re-Enrollment Form -- this form is submitted by student to the academic
advising office which forwards it to the dean for approval.
- Scholastic Standing Committee Forms -- this is the form to petition to drop a course without penalty (the link title should be changed accordingly). The student's course instructor and faculty advisor each completes and signs one part of this form; it is then submitted to the Scholastic Standing Committee for approval (or disapproval).
- Permission Form for an Academic Course Overload – if a student has declared a major, this form is approved and signed by the departmental advisor, department chair, associate dean, or dean (depending on the no. of credits and cumulative gpa -- see form for details). If a student has not declared a major, the form is signed by the associate dean.
- Incomplete Grade Extension Form -- this form requires 3 signatures: instructor, department chair, and associate dean.
- Prerequisite Override Form -- this form may be signed by the student's faculty advisor, an academic advising dean, course instructor, department chair, or summer session director (for summer courses).
- Revision of Transfer Credits (Department Evaluation) -- this form is for transfer credit evaluation and is signed by either department chair or associate dean.
- Rutgers-Camden Withdrawal Form -- this form is submitted if student wishes to withdraw from school; it is signed by the financial aid office and Mary Beth Daisey as dean of students or one of the academic advisors.
- Registration Form -- this form is filled out by student during transfer registration days and can be used by students to plan their schedule; it is signed by the student's advisor.
